The gameplay changed slightly from the original Rabio Lepus: instead of being divided into three levels, the spaceship and asteroid were each a single level containing three bosses. A few of the enemies differed as well, and Lepus was no longer playable. The intro showing the royal family being kidnapped was removed, as well as the ending showing a statue of Rabio/Lepus. The game's manual instead provided the backstory in the form of a comic. In addition, the graphics showing the royal family were completely reworked. The character designs of the king Taro Kitashirakawa and princess Komomo Kitashirakawa were not changed significantly, but Komomo's little sister Kotomi Kitashirakawa has a much different design. She appears younger than in Rabio Lepus, and she wears a full-body bunny costume like the king's rather than a Playboy-bunny-esque leotard with a bunny ears headband. Rabio Lepus Special came with a manual featuring all-new artwork, including the comic giving the backstory of the game and illustrations of some of the enemies.
